The correct diagnosis will lead to the correct treatment. Know Your Knee! The knee is the largest joint in the body. Its also one of the most complicated. The knee joint is made up of four bones that are connected by muscles, ligaments, and tendons. The femur (large thigh bone) interacts with the two shin bones, the tibia (the larger one) located towards the inside and the fibula (the smaller one) located towards the outside. Where the femur meets the tibia is termed the joint line. The patella, (the knee cap) is the bone that sits in the front of the knee. It slides up and down in a groove in the lower part of the femur (the femoral groove) as the knee bends and straightens. Ligaments are the strong rope-like structures that help connect bones and provide stability. In the knee, there are four major ligaments. On the inner (medial) aspect of the knee is the medial collateral ligament (MCL) and on the outer (lateral) aspect of the knee is the lateral collateral ligament (LCL). The other two main ligaments are found in the center of the knee. These ligaments are called the anterior cruciate ligament (ACL) and the posterior cruciate ligament (PCL). They are called cruciate ligaments because the ACL crosses in front of the PCL. Other smaller ligaments help hold the patella in place in the center of the femoral groove. Two structures called menisci sit between the femur and the tibia. These structures act as cushions or shock absorbers. They also help provide stability for the knee. The menisci are made of a tough material called fibrocartilage. There is a medial meniscus and a lateral meniscus. When either meniscus is damaged it is called a “torn cartilage”. There is another type of cartilage in the knee called hyaline cartilage. This cartilage is a smooth shiny material that covers the bones in the knee joint. In the knee, hyaline cartilage covers the ends of the femur, the femoral groove, the top of the tibia and the underside of the patella. Hyaline cartilage allows the knee bones to move easily as the knee bends and straightens. Tendons connect muscles to bone. The large quadriceps muscles on the front of the thigh attach to the top of the patella via the quadriceps tendon. This tendon inserts on the patella and then continues down to form the rope-like patellar tendon. The patellar tendon in turn, attaches to the front of the tibia. The hamstring muscles on the back of the thigh attach to the tibia at the back of the knee. The quadriceps muscles are the muscles that straighten the knee. The hamstring muscles are the main muscles that bend the knee. Bursae are small fluid filled sacs that decrease the friction between two tissues. Bursae also protect bony structures. There are many different bursae around the knee but the ones that are most important are the prepatellar bursa in front of the knee cap, the infrapatellar bursa just below the kneecap, the anserine bursa, just below the joint line and to the inner side of the tibia, and the semimembranous bursa in the back of the knee. Normally, a bursa has very little fluid in it but if it becomes irritated it can fill with fluid and become very large. Is it bursitis… or tendonitis…or arthritis? Tendonitis generally affects either the quadriceps tendon or patellar tendon. Repetitive jumping or trauma may set off tendonitis. The pain is felt in the front of the knee and there is tenderness as well as swelling involving the tendon. With patellar tendonitis, the infrapatellar bursa will often be inflamed also. Treatment involves rest, ice, and anti-inflammatory medication. Injections are rarely used. Physical therapy with ultrasound and iontopheresis may help. Bursitis pain is common. The prepatellar bursa may become inflamed particularly in patients who spend a lot of time on their knees (carpet layers). The bursa will become swollen. The major concern here is to make sure the bursa is not infected. The bursa should be aspirated (fluid withdrawn by needle) by a specialist. The fluid should be cultured. If there is no infection, the bursitis may be treated with anti-jnflammatory medicines, ice, and physical therapy. Knee pads should be worn to prevent a recurrence once the initial bursitis is cleared up. Anserine bursitis often occurs in overweight people who also have osteoarthritis of the knee. Pain and some swelling is noted in the anserine bursa. Treatment consists of steroid injection, ice, physical therapy, and weight loss. The semimembranous bursa can be affected when a patient has fluid in the knee (a knee effusion). The fluid will push backwards and the bursa will become filled with fluid and cause a sensation of fullness and tightness in the back of the knee. This is called a Bakers cyst. If the bursa ruptures, the fluid will dissect down into the calf. The danger here is that it may look like a blood clot in the calf. A venogram and ultrasound test will help differentiate a ruptured Bakers cyst from a blood clot. The Bakers cyst is treated with aspiration of the fluid from the knee along with steroid injection, ice, and elevation of the leg. Knock out knee arthritis… simple steps you can take! Younger people who have pain in the front of the knee have what is called patellofemoral syndrome (PFS). Two major conditions cause PFS. The first is chondromalacia patella. This is a condition where the cartilage on the underside of the knee cap softens and is particularly common in young women. Another cause of pain behind the knee cap in younger people may be a patella that doesnt track normally in the femoral groove. For both chondromalacia as well as a poorly tracking patella, special exercises, taping, and anti-inflammatory medicines may be helpful. If the patellar tracking becomes a significant problem despite conservative measures, surgery is need. While many types of arthritis may affect the knee, osteoarthritis is the most common. Osteoarthritis usually affects the joint between the femur and tibia in the medial (inner) compartment of the knee. Osteoarthritis may also involve the joint between the femur and tibia on the outer side of the knee as well as the joint between the femur and patella. Why osteoarthritis develops is still being scrutinized carefully. It seems to consist of a complex interaction of genetics, mechanical factors, and immune system involvement. The immune system attacks the joint through a combination of degradative enzymes and inflammatory chemical messengers called cytokines. Patients will sometimes feel a sensation of rubbing or grinding. The knee will become stiff if the patient sits for any length of time. With local inflammation, the patient may experience pain at night and get relief from sleeping with a pillow between the knees. Occasionally, locking and clicking may be noticed. Patients with osteoarthritis may also tear the fibrocartilage cushions (menisci) in the knee more easily than people without osteoarthritis. So how is the arthritis treated? An obvious place to start is weight reduction for patients who carry around too many pounds. Strengthening exercises for the knee are also useful for many people. These should be done under the supervision of a physician or physical therapist. Other therapies include ice, anti inflammatory medicines, and occasionally steroid injections. Glucosamine and chondroitin supplements may be helpful. A word of caution… make sure the preparation you buy is pure and contains what the label says it does. The supplement industry is unregulated… so buyer beware! Injections of the knee with viscosupplements lubricants- are particularly useful for many patients. Special braces may help to unload the part of the joint that is affected. Arthroscopic techniques may be beneficial in special circumstances. Occasionally, a surgical procedure called an osteotomy, where a wedge of bone is removed from the tibia to even things out, may be recommended. Joint replacement surgery is required for end stage knee arthritis. Research is being done to develop medicines that will slow down the rate of cartilage loss. Targets for these new therapies include the destructive enzymes and/or cytokines that degrade cartilage. It is hoped that by inhibiting these enzymes and cytokines and by boosting the ability of cartilage to repair itself, that therapies designed to actually reverse osteoarthritis may be created. These are referred to as disease-modifying osteoarthritis drugs or DMOADs. Genetic markers may identify high risk patients who need more aggressive therapies. Newer compounds that are injected into the knee and provide healing as well as lubrication are also being developed. And finally, less invasive surgical techniques are also being looked at. Recent technological advances in mini knee replacement look very promising. Brockages Not All That Common

By Brad Hall Another term that has some of the newer error coin collectors scratching their heads is brockages. The term is certainly not self defining that much is certain and it points to a very rare occurrence in the minting process. This is one of those errors that is extremely hard to locate and above all else, very valuable. Brockages occur when two coins are involved in the same retaining collar. First off you need a coin that is already struck and it is stuck in the retaining collar. Then a new coin blank is inserted. Once this occurs the dies come in for the strike and both of the coins are affected by said strike. What you end up getting is a coin blank with a mirror image of the already struck coins impression incused into the surface. This is basically a coin that was struck backwards and has no raised impression on the brockages side of the coin. For newer coinage it is a fairly rare occurrence to find this type of error. Because of the new counter measures that the mint has in place the operator is alerted when a coin fails to eject from the retaining collar. When this occurs the operator shuts down the machine and removes the coin and checks for defects in the machine. The older coins are more common to find brockages. Before a lot of stress was being put on the mint about the errors that were being produced many of this type of coin error were released. However, when it comes to error coins the older the coin the more valuable the coin. So brockages in the older mints do command a much higher price than one would expect. Brockages are not unheard of though when it comes to the newer coinage. One of the most significant examples recently sold at auction and it was a nice New Jersey state quarter that had complete reverse brockage of the reverse image. All in all you will find that most of the brockages that appear these days are not full impressions, but just partly so as the blank attempts to fall away because the retaining collar is full. These examples still have a fair amount of value and can command a hefty price. The History of Facelifts

During these early forms of the facelift procedure, the only concern was repositioning the skin so that the appearance of wrinkles and creases would be corrected. Facelifts were performed by making incisions, pulling back the facial skin, removing excess tissue, and closing the incision. In the 1970s, doctors realized that although repositioning the skin was key in providing the youthful facelift results people were looking for, the deeper tissues and muscles also had to be repositioned to provide the excellent, long-lasting results people are looking for. Although the skin-only facelift is essentially a technique of the past, says Dr. Douglas Hendricks in Newport Beach, California, the facelift procedures of today would not be in existence without the knowledge gained from the early versions of the facelift. In 1974, doctors began repositioning the Superficial Musculoaponeurotic System (SMAS). The SMAS is the layer of muscle and tissue below the skin, and up until 1974, doctors were performing facelifts by only repositioning the skin. By repositioning both the skin and the SMAS, plastic surgeons are now able to provide patients with wonderful, long-lasting, and natural looking results. A facelift can reduce the signs of aging like wrinkles and creases in the cheeks, jowls, and neck. Facelifts today begin with incisions that are concealed within the hairline. From these incisions, the skin is separated from the muscles and tissue beneath. The SMAS layer is then tightened to provide the youthful appearance in the lower face, mid-face, and neck. After the SMAS layer is tightened, the skin is pulled back to reduce lines and wrinkles. Excess skin and fat is removed, and the incisions are closed. How To Stay Vital - The First Easy Step

By Christopher Walker A significant cause of tiredness is Sleep Deprivation this article presents a formula to overcome it. Sleep deprivation affects over 50% of the western world. Its effect is devastating. Changing mood, concentration, happiness, health, vitality and emotional balance. Sleep deprivation is a hidden gremlin that lurks behind so many of our day to day problems. Depression, exhaustion, anger, frustration, laziness, mental fatigue, poor decision making, anxiety, fear, relationship breakdown, impotence, sexual performance drop, abuse, violence, alcohol addiction and obesity. Sleep deprivation affects over 50% of the people you know, and yet, they will say their problems are not associated with poor sleep or the lack of it. 50% of your friends will be visiting chiropractors, health food shops, health farms, yoga classes, exercise programs, beauticians, doctors and taking magic herbs to deal with issues that at their root, come from sleep deprivation. How long we need for sleep is an individual thing. Some people are able to get away with 4 hours a night, others, like me, need a solid 8 hour sleep to manage a healthy life sustainably. Some need more. My old friend Athena Starwoman (RIP) used to sleep 18 hours a day, and wake up ready to do her astrology charts. She said she worked best in her sleep. The social circumstances surrounding our increasing sleep deprivation are: a. Computers and telephones stimulate the nervous system upsetting bodyrythms. b. Television keeps us awake and stimulated when we should be getting ready for sleep c. Higher than normal levels of mental complexity (we are stuck in our head all day) d. Low physical demands we are not physically tired at night, so urge to sleep not high e. We are dehydrated and we eat too much too late of an evening f. Tea, coffee and sugar lasts 4-5 hours to falsely stimulate blood pressure (keeps awake) g. We have lost the art of the bedroom ritual. h. All night dance parties are becoming popular (it takes a week to recover from that) These are the reasons we dont sleep as much as we need to. But more important than the hours of sleep, is the quality. So much research has been done on the quality of sleep, and the effect of sleep that is not deep. The results are astonishing. 12 hours of poor sleep is not even equivalent to 2 hours of deep REM sleep. This explains why some people only need a few hours sleep and others need more. The quality of the sleep makes the difference. Sleep quality is affected by many things. To maximize the quality of your sleep consider the following. 1. Create two beds side by side, one for you and one for them. The beds can be joined but because temperature affects sleep more than any other single thing, try to manage your own heat (bed clothes and blankets) independently. Women typically need 10 degrees more heat to sleep comfortably at night than men. 2. Be light. Dont eat or drink within 2 hours of going sleep. Preferably 3. This is the cause of many people staying up late. They eat late and cant sleep. So they stay up and watch TV and then eat because they cant sleep. Etc 3. Sleep ritual. A healthy person takes between 15 and 20 minutes to fall asleep at night once they are in bed. Faster than that reveals over tired. Longer than that reveals hyperactive mind. Hyperactive minds come from working near bedtime, watching TV near bedtime, and worst of all, being exposed to computer screens within an hour of going to bed. Bed ritual means the journey to sleep is a half hour gradual slow down with minimal talk and some personal ritual. 4. Slow down, enjoy the peace. Reading, watching TV in bed and talking too much before bed (especially on the phone) causes sleep quality to drop below the threshold of 50% value (need twice as much) 5. Do physical exercise. You body must want rest. Food fed to your body turns to fat if you dont use it. Fat keeps you awake at night because it wants to burn. There must be at least a 40 minute exercise session in which your heart rate is above 60-70% MHR. 6. Cool your Mind. A person who is not looking forward to tomorrow is in resentment. A person who is hyperventilating about tomorrow is in infatuation. Both are hyper and this causes the heat in the mind and blood that keeps people awake, or out of depth of sleep. The best thing to do before you go to lie in bed is prepare a list of tomorrow. If it is so exciting that you are wishing it would come now, then think of some negatives that will come tomorrow. If you are resenting tomorrow, think of the good news, or change the plan. 7. Lie on your back. It takes 6 weeks to develop the habit of sleeping on your back, but it improves the quality of your sleep by affecting your breathing, body relaxation and more. Use the right height pillow and dont cross your legs. (of course if you snore then ) 8. Create the Thank you Attitude. Sleep is such a potent part of our lives. It deserves the same effort as work and love and health. We sleep for 30% of our existence on this earth. Why not make it amazing? To do so, I suggest the appreciation ritual, alone or with lover. The appreciation ritual is the process of going over the past 24 hours and mentally, or quietly with partner, listing everything you can be thankful for, for that whole period. Then add the whole week, the year and finally your life. If you do this in bed, youll sleep more than most and better. Dealing with cause The first step in maintaining vitality of life/. These recommendations are very important. They really help your quality of sleep but they really come naturally to us when we understand the gift of stillness. The person who does not like their own company will not sleep well. Guilt, shame, anger, victim hood and more emotions make it hard to be alone with yourself. So good sleep is one of the best rewards for a good life, a real life. Stillness is the art of enjoying your own company. Without proving, doing, achieving or wanting, without not wanting or not proving, being real, being yourself. Just stillness. The art of just lying there in bed, together with your lover or alone, in stillness, is a magnificent gift. One that we all have, naturally, if we live in an authentic way. Being yourself? Who is that? Which self? This is the deepest question you can ask. Which self you want to be. The smaller you define self i.e. the more accurately you define self, the more ego you have. To say I am nothing for example, is the height of egotism. To say I am everything is even greater egoism. These definitions of yourself just promote drama and subsequent sleeplessness. How can you sleep deep when you are being someone. If there is a sense of self, that means you are stuck in your ego, unable to surrender. Sleepless people are stuck in their ego. They might say they are godly but really, they dont understand the difference between emotion and love. To fall asleep is to fall in love. That is why sex happens so often at night, it puts you in the mood for sleep because it can, or should at least, put you in the mood for love. So when we talk about insomnia, or lack of sleep, poor preparation for sleep, or a person who stays up because they dont want to go to bed, the real reason is they just dont feel lovable, and therefore cant find stillness. So, theres the first of the three simple steps to long life vitality. Find stillness. Finding stillness requires that you learn the art of Stepping away Stepping away means being nothing. Just being nothing. It means walking for no reason. Kissing for no reason. It means sitting under a tree for no reason. Reasonlessness if that is a word. Its getting harder and harder to leave without reason. Everything is reasonable. Reason is what children are taught is behind everything. Reason for this and a reason for that. Why did you do that? I dont know oh, come on, there must be a reason When we always answer our childrens questions of WHY? with a reason, they loose the essence of reasonlessness. Just the art of being nothing, not being. Children who cant sleep are children with only reasons. There is no reason to fall asleep, we dont need one. It is just time to surrender, and be ourselves. If we are taught always to be someone, do someone, act someone or become someone then sleep is hard to achieve, because in bed, no matter where we are, we are with ourselves. We are us. Reasonlessly. Nature is the greatest teacher of stillness. If we go outside in nature and sit in a park, very soon we get restless. Because we arent the most important thing in the park. We are looking at the park, we are a part of the park but we dont own, control, manage, plan, design, care for, possess or even reject the park. We are in the park but not the dominant force. So we get restless sometimes we get restless because there is too much stillness. Or we become overwhelmed with the beauty, too much beauty overwhelms us, our sense of self is lost. The best sleep comes to a body thats done a days exercise, to a mind thats thankful, to a heart thats loving. But the deepest sleep of all comes to a person who is in love with stillness. Little Mistakes That Keep You Unemployed

By Kevin Donlin If your job search is dragging on and on, you might want to look in the mirror. Because the person looking back may be sabotaging your efforts. Do you make the following mistakes in your job search? If so, stop now. And start getting more calls for job interviews. Mistake #1) Not Following Up If you fire off resumes without checking to see if employers get them, and fail to keep in touch until a hiring decision is made, your dream job might go to someone less qualified, but more persistent than you. Here’s how one of my clients, Mike M. from Boston, followed up right. And got a great new job. “After going to about 20 interviews, I found that following up on resumes submitted via email is very important. Probably a third of my interviews were obtained within a day or two after a follow-up,” says Mike. How did he do it? “I sent my resume and cover letter again via email, and included one or two wow’ points about myself in the email message.” Here’s an example of a follow-up email Mike sent: Hello, I would like to follow up on the status of my resume submission for the Forecast Analyst position, as there is a very strong match between the position requirements and my skills and experience. [I've attached my cover letter and resume again for your convenience.] As I have a strong desire to re-enter the consumer products industry, I would again welcome the opportunity for an interview. With a track record of delivering up to $1 million in annual cost savings through accurate demand forecasts and supply-chain planning, I’m confident I could bring the same performance to ABC Co. Sincerely, Mike M. Nothing earth-shattering there. Just plain, smart persistence that set Mike apart from other candidates. And it worked — he got hired. You can do this, too. Mistake #2) Not Thinking Things Through Last week, I tried calling “Jill” (not her real name) in reply to a job search question she had emailed me. After 5 rings, Jill didn’t pick up the phone … and neither did an answering machine. So there was no way to leave a message. What if I had been a busy hiring manager calling to set up a job interview? I would have passed on Jill and phoned the next candidate. So I emailed Jill to suggest that she get an answering machine. Her reply: “I do not have access to an answering machine because of my limited income.” To which I was tempted to answer: “Your income may be limited because potential employers can’t reach you!” Think it through. Would you go trout fishing without a creel? Of course not — how would you carry home the fish you caught? So why send out resumes if you don’t have an answering machine to “catch” calls from employers? Especially when you can buy one at Radio Shack for less than $30 — about what you’d pay for a couple of large pizzas. That’s a fair trade in my book. Question: Are there any “leaks” in your job search? Are employment leads slipping through your fingers? Think it through now. Mistake #3) Not Getting Input From Others After reading more than 10,000 resumes over the years, I’ve found about 90% of job seekers are suffering from an “affliction” that keeps them unemployed. What is it? Let me illustrate with a story … You’re eating dinner and your mom asks: “Could you get me the salt?” You walk into the kitchen and, after searching all the cupboards, you call out in a frustrated voice, “I can’t find the salt!” Then your mom walks in, picks the salt up off the shelf in front of you and says, “Look! It was right here in front of your nose!” In psychological terms, you have a “scotoma.” (Say: ska-toe-ma.) I have discovered that most job seekers suffer from “resume scotoma.” They unknowingly overlook obvious errors in their resume … until someone, like me, points them out. Here’s an example. “Jerry” from New York sent me a resume used to apply for jobs as a logistics manager. Yet, most of his resume was filled with irrelevant facts (“Honorably retired from the United States Marine Corps”), empty assertions (“Extremely dependable”) and unnatural language (“Excel at directing a cohesive staff in the successful attainment of objectives.”) Jerry had overlooked a host of flaws. When I asked if he had shown his resume to anyone for input, his response was a predictable, “No, I never thought of that.” To avoid “resume scotoma” — and get hired faster — show your resume to at least 3 friends before sending it to employers. Stress Can Help Make You Fat

By Jim O’Connor Do you know by not using proper stress management techniques or stress relief strategies you could be contributing to fat gain? There is a condition called stress induced obesity which promotes a stress producing hormone called cortisol. Let’s take a closer look at how stress, cortisol, and fat promotion can make your waist, hips, and buttocks larger. Stress is a word you hear often these days. Due to the extremely fast paced world we live in, greater demands are placed upon us, leading to greater stress levels which are higher than ever seen before. People desperately attempt to juggle professional careers, family, and free time, all while fighting traffic jams, social obligations, financial pressures, and even natural disasters. All of this results in being “stressed out.” Demands are consistently being placed upon us from different directions, sometimes occurring at the exact same time. I constantly hear the words “if there were more hours in the day.” Sound familiar? Stress is defined as any event in which environmental demands, internal demands, or both tax or exceed the adaptive resources of an individual, social system, or tissue system. How many times have you heard the controversial ads for Cortislim or Relacore? Both are suggested to block cortisol, a stress hormone that gets your body ready to respond to a threat, and has been associated with fat promotion. Depending upon the level of threat, the brain regulates the amount of cortisol excreted from the adrenal gland. If the risk is huge, like a bear chasing you, a large amount of cortisol is released into the blood stream. However, if you are late for a very important appointment, and rushing vigorously, a smaller amount of cortisol is released. The state of readiness, the “fight or flight” response, which promote the hormone cortisol, has been associated with increasing fat storage, and cardiovascular effects such as faster heartbeat, higher blood pressure, constricted blood vessels, and a release of blood sugar. Energy regulation and mobilization are also two crucial functions of cortisol. Personality type, which includes the way you interpret stressors, can play a key roll in cortisol release, according to research studies. Suppose you are considered a personality type D, which exhibits negative emotions, pessimistic behavior, and don’t share emotions. In addition to your interpretation of stressful situations, you also lose your job. You would now be considered having the cortisol throttle wide open, stuck in the “on” position. Over the long haul, along with the heart effects mentioned about, fat gain can be promoted by the aggressive circulation of cortisol in the blood stream. According to research, tissue cortisol concentrations are controlled by a specific enzyme, cortisone, that converts inactive cortisone to active cortisol. Studies show that the gene for this enzyme shows up more in obese conditions. It is shown that deep fat, surrounding the stomach and intestines have more of these enzymes compared to fat just underneath the skin. Thus, the high enzyme levels in the deep fat tissue surrounding the abdomen may lead to obesity due to greater levels of cortisol being produced at the tissue level. It is also noted that deep abdominal fat has greater blood flow and four times more cortisol receptors compared to subcutaneous, or surface fat. This, in effect, may increase cortisol’s fat accumulation and fat cell size enlarging effect. Another potential link between stress, and cortisol involves the appetite. Animal and human studies have shown an increased appetite, cravings for sugar, and weight gain when cortisol injections were administered. Individuals with high circulating levels of cortisol tend to consume more foods high in sugar, and fat. This may occur because cortisol directly effects food consumption by binding to receptors in the brain. This can stimulate an appetite craving sugar, and fat. Due to all the negative influences mentioned above, resulting from continual high stress levels effecting circulating cortisol, it is a good idea to manage your stress level well. Please be aware that Cortislim, and Relacore do not appear to be the answer. The federal government has cracked down on products like these for stating false and unsubstantiated product claims. More research needs to be done on these products before being considered a viable solution. A much more effective solution is watching your caloric intake, while participating in a consistent exercise program. You should also include stress reduction methods such as meditation, biofeedback, visualization, and/or yoga. Work hard at exercising, keep a positive mental attitude, and incorporate as many stress reducing techniques as you possibly can. It will benefit your waistline, as well as overall health.
